Instructions
Thank you for your interest in Leadership Jefferson. Please complete each section fully, limiting answers to the spaces provided. Please make every effort to complete this application electronically. Your application will be reviewed by the Leadership Jefferson Selection Committee.
A non-refundable application fee of $35.00 is required with the submission of your application. Tuition for each applicant, upon acceptance to the program, is $1100 for Jefferson Chamber members $1350 for non-members. Do not send the full tuition until you are notified of your acceptance in the program. All applicants will be notified no later than September 5, 2011.

Application Checklist
1. Check your calendar for availability on the program dates: • Overnight Opening Retreat—Friday, September 30, 2011 beginning at 8:00 a.m. and staying overnight until noon on Saturday, October 1, 2011, at Carter Plantation in Springfield, Louisiana. • Seven Monthly Sessions (maximum of 2 absences allowed) o Quality of Life October 18, 2011 o Healthcare November 8, 2011 o Economic Dev December 13, 2011 o Education January 10, 2012 o Diversity February 14, 2012 o Governance March 13, 2012 o Community Inv April 10, 2012 • Overnight Closing Retreat—Friday, May 11, 2012 beginning at 8:00 a.m. and staying overnight until noon on Saturday, May 12, 2012. Location: TBD. • Graduation for the program is scheduled for May 23, 2012 at 6:00 p.m. with reception followed by dinner.
In addition to these dates, each class is asked to participate in a service based project of mentoring an in-need school student. As a mentor each class member will be giving back to the community we live in. This mentoring program runs concurrently with the Leadership Jefferson program and will be beneficial and rewarding to the class. The program requires roughly an hour of each class members time per week and can be scheduled at the class member’s preference. Class members will individually participate in this program. Leadership Jefferson will arrange an orientation to the program after the selection of this year’s class.
2. Complete Application Form (please make sure that the application is signed by you and your employer, if your employer is sponsoring your enrollment.
3. Process check made payable to Jefferson Chamber of Commerce for Application Fee of $35.00 (or supply information for credit card payment).
4. Submit application and payment (if by credit card) by email to Ronnie Slone, Program Director at Ronnie@theslonegroup.com.
